This plan was designed with your needs in mind and to help protect you and your family from costs associated
with unexpected illnesses and accidents. The plan includes coverage such as life insurance, dental,
prescription drugs and long term disability. Please take the time to enroll yourself as well as any dependents

Employees are eligible to receive coverage on the first day of the second month following three hundred (300)
hours worked. For example, if you complete 300 hours worked on November 15th, your coverage will
commence on January 1st. It is the responsibility of the employee to complete the enrollment form for the
benefit plans, which is a condition of coverage.
